Spiral Insana is an album by Nurse With Wound. In the book England's Hidden Reverse, author David Keenan describes the album as one of NWW's "most powerful records" but principal member Steven Stapleton advised Keenan that the making of "Spiral Insana" was particularly difficult

Spiral Insana is one continuous piece of music, indexed over three tracks (on the compact disc), with 20 track titles listed on the cover. Such is Steve Stapleton's Nurse with Wound. And it's just as well, because the music inside, more ambient and user-friendly than on other NWW outings, is still just as surreal and avant-garde as the rest of the catalog

Lemana
Spiral Insana, considered by Steven Stapleton to be the proper follow-up to 1982's Homotopy to Marie, finds Nurse With Wound charting more mellow and spacey avenues than before. The warm but melancholic synth motif that repeats throughout the first side-long suite gives the album a much more traditionally musical feel than anything released under the name previously, and adds considerably to the album's reputation of being one of the less challenging NWW records. Melodic synth pads aside, the rest of the album is effectively one long piece, a single sound collage with returning themes (notably a number of organ chords that, despite the album's relatively sedate mood, still manage to creep up out of nowhere; a buzzing rhythmic loop also features heavily) and individual sections. Nurse staples such as jaunty jazz piano, choirs, jangling percussion and disembodied voices appear, while the usual electronic tones and feedback are dialled back to form relatively smooth textures rather than harsh blasts. A section for percussion and acoustic guitar midway through the second section of the album lends a rural feel to proceedings, one which is echoed in the CD's bonus track 'Nihil'. The track fuses several elements of the original album with birdsong, farmyard sounds, rutting stags and other natural sounds. It's likely that 'Nihil' is from the 1990s and was created especially for the CD edition of the album, but it rounds out the album nicely, ending on a mellow note which ties in nicely with other late '80s NWW records like A Missing Sense and Soliloquy for Lilith. Spiral Insana is an album that requires patience, and its use of repetition and recurring themes means it's less suited to people after the crazier approach found on albums like Homotopy to Marie and Sylvie & Babs. Given some time, however, it is a rewarding listen, with a more welcoming atmosphere than most Nurse With Wound albums.
